Biliary - Choledocholithiasis, Giant, Treated with DACP and EHL

Comments: This 87 year old woman with right upper quadrant pain and abnormal liver function tests was referred for ERCP. This 2 by 3 cm stone was seen in the distal common bile duct. Note the failure to trap this giant stone in the large lithotripsy basket. Therefore, EHL was used. The probe is positioned adjacent to the stone, and fired. Bleeding may be seen if the discharge occurs too close to the mucosal wall. The bleeding is usually mild and self limited. Once the stone is fully fragmented, the pieces are removed using a balloon and basket. A stent was placed for two weeks to allow the edema to subside.
