Biliary - Ascaris Lumbricoides

Comments: This 30 year old female resident of rural China presented with symptoms suggestive of biliary colic and nausea and vomiting of one day’s duration. She had no fever or jaundice. A right upper quadrant ultrasound was performed which demonstrated a filling defect in the common bile duct. An urgent ERCP was performed in Beijing. The ampulla is noted to be patulous, no sphincterotomy or balloon dilation was needed. Injection of the bile duct suggested the presence of a parasite. Cannulation is achieved with a biliary basket. A live Ascaris is retrieved from the bile duct. Due to the very large size of this worm, the organism fractured during extraction and was removed in two pieces. Other sites of extraintestinal ascariasis include the gallbladder, intrahepatic bile ducts, pancreatic duct, and occasionally the airway. The most common extraintestinal site is the common bile duct.
